What we do, and why we are telling you
To display before-and-after photographs side by side, the practice uses software that identifies reference points on the face — for example the corners of the eyes and mouth — so that the two images line up and your face does not appear to shift between them. Illinois law excludes ordinary photographs from the definition of a biometric identifier, and the practice’s position is that this alignment step does not create one. Out of caution, and so that you are fully informed, we are telling you about it and asking for your permission anyway.
What is collected
Coordinates describing the position of facial reference points, derived from your photographs.
Why
Solely to align before-and-after images with each other. It is not used to identify you, and it is never used to search for, match, or recognize you in any other image.
How long it is kept
The coordinates are used at the moment the images are prepared and are then discarded. They are not stored, and in no case are they retained beyond the earlier of the purpose being satisfied or three (3) years from your last interaction with the practice.
Who it is shared with
No one. The alignment is performed by the practice and the data is not sold, leased, traded, or otherwise disclosed to any third party.
Your consent
Consent to this alignment step is taken in writing, on its own initial line of the new patient consent form, separately from your consent to treatment and separately from any permission you give for your photographs to be published. Declining it does not affect your care.
